SG Woman Accidentally Injected With 5 Doses of Pfizer Covid-19 Vaccine After Human Error

A human error in the administration of the Pfizer-BioNTech Covid-19 vaccine in Singapore led to a local woman to be injected with the equivalent of five doses of the vaccine instead of only just one. As reported by Channel News Asia, the woman, an employee at the Singapore National Eye Centre (SNEC) received the dosage on January 14 during an exercise conducted by the centre for its staff.

After the error was detected, the vaccination exercise at SNEC duly stopped and all of its employees were asked to be vaccinated at the Singapore General Hospital (SGH).

The error was discovered minutes after the woman was vaccinated and senior doctors were alerted immediately. The woman was then assessed and found to not have suffered any adverse reaction or side effects. However, the SNEC employee was admitted to SGH for observation just in case anything were to happen.

She was discharged two days later after her condition remained stable throughout the period. A review of the error shows that a lapse in communications among the vaccination team saw one staff member had mistaken an undiluted dosage of the vaccine for an already diluted dosage for administration. The staff that was in charge for diluting the dosage had been called away to attend to other matters during the preparation of the vaccine.

Well, thankfully the woman is okay after receiving an equivalent of 5 doses of the vaccine. Well, seeing how the vaccine itself is hard to get, let’s hope that when it arrives in Malaysia, mistakes such as this would not happen during the vaccination process in our country.
